RN Case Manager – (Fayetteville, AR)
Community Clinic is seeking a full-time RN Case Manager to work directly with its medical providers. This position provides ongoing support and expertise through comprehensive assessment, planning, implementation and evaluation of individual patient needs to improve outcomes and overall health. A current RN license is required. A minimum of 2 years of utilization review/case management experience is preferred. Bilingual Spanish/English is a plus. Physical Therapist (Rogers, AR)
This position oversees the day to day activities of the organization’s physical therapy program while also providing direct care to patients. Key responsibilities include: providing leadership for the physical therapy program across locations, administering medically prescribed physical therapy, determining treatment plans in consultation with physicians or by prescription, and evaluating patient outcomes. Must be a graduate of an accredited physical therapist education program and be licensed to practice in the state of Arkansas. Normal work hours are Monday – Friday. Part-time Assistant Project Coordinator
Community Clinic is seeking a Part-time Assistant Project Coordinator. This position assists with research, development and submission of work plan goals and activities of the Drug Free Communities program. Part-time, 15-20 hours a week with occasional weekends.
