Discount Programs

We believe that quality, comprehensive health care should be available to everyone. That’s why we offer all of our services on a sliding fee scale according to each patient’s financial situation, determined by income and household size.

To determine what discount you are eligible for, please bring Proof of Income (POI) to your first appointment.

Accepted forms of POI

  • Two (2) consecutive paycheck stubs or payroll statements for the current year.
  • W-2 form or last year’s state, federal, or tribal tax return (for the most current year).
  • TEA, SSI, or Social Security statement of benefits or current earning statement.
  • Unemployment or Workman’s Compensation statement of benefits.
  • Veterans Administration or Railroad or other retirement statement of benefits.
  • Child support or divorce decree with alimony information.
  • Employer verification form, completed by employer.
  • SNAP (Food Stamp program) statement of benefits with income shown.

A “Good Faith Estimate” provides you with an estimate of the expected cost for non-emergency medical items and services. This includes costs related to tests, prescriptions, equipment, and hospital fees. Your healthcare provider is required to provide this estimate in writing at least one business day prior to your scheduled service. If you receive a bill that exceeds the estimated amount by $400 or more, you have the right to dispute it.

You Have the Right to Receive a Good Faith Estimate Explaining How Much Your Medical Care Will Cost

Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your healthcare provider for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
