Community Clinic serves as a medical home for patients by providing primary care to prevent illness, promote wellness, and manage chronic disease. As a medical home, we help patients improve their health care outcomes and decrease their health care costs – reducing the need for avoidable, costlier care such as emergency department visits, hospitalizations, and the progression of early symptoms into more serious chronic conditions.
Community Clinic is a Patient Centered Medical Home (PCMH). As a PCMH, we follow the medical practice model that maintains the following core values:
- Comprehensive Care
- Coordinated Care
- Accessible Services
- Quality and Safety
Community Clinic is a Federally Qualified Health Center (FQHC). To maintain this designation, we operate under the highest standards of accountability in the medical field and regularly assess outcomes in order to enhance patient experience, improve health results, and reduce per capita costs.
